A skilled nursing facility provides 24-hour skilled nursing care to patients under the orders of a licensed physician. Patients can be admitted from a hospital, another post-acute care provider, or directly from an emergency room or home.


Patients typically require a high level of medical care and assistance. Skilled nursing facilities offer a range of services, from short-term rehabilitation to nursing care, for those patients who are recovering from an injury, illness, or surgery. The goal is to help patients reach their maximum potential before they return home.


For those patients who require long-term or a respite stay, skilled nursing facilities can provide ongoing care for individuals with complex medical conditions or those with high-care needs. Hospice and palliative care can also be provided in this setting. An assessment by our Community Liaison can be completed at the hospital, post-acute setting, or home to assist with admission to our client facilities.
